Memorandum of Understanding and Agreements (MUA) Form

• Registration

Principal Investigators must fill out an MUA for all research and teaching activities involving the use of regulated recombinant DNA and biohazardous materials.

Cornell's web based system for managing the submission, review and approval of MUA's can be accessed here. All researchers have access to the electronic system and can create new MUA's and modify/view existing MUA's by using their Cornell Net ID. Appropriate forms will be generated based on investigator indications of the type of work being conducted, i.e. work with materials EXEMPT from the NIH Guidelines; work with materials that are NON-EXEMPT from the Guidelines; work with BIOHAZARDOUS materials. Guidance documents exist within the MUA forms to assist investigators.

To submit an amendment to an existing MUA, access the electronic submission system, using your Cornell Net ID and select the appropriate MUA to amend. Select the option to create an amendment.

Annual Questionnaire

Email notifications will be sent out alerting investigators that an update to the MUA is due. Access the system using the link provided in the email notification or access the system's main page and select the MUA to update.

Cultures of infectious agents, mammalian cell lines, and sharps and non-sharps items in contact with infectious materials (e.g., gloves, plastic ware, hypodermic syringes and needles) must be disposed as Regulated Medical Waste. Investigators working with infectious or biohazardous materials, or using high risk procedures (centrifugation, sharps, aerosol generating procedures) should develop and implement standard operating procedures. Sample procedures for guidance are provided.

Investigators are responsible for developing/implementing a Chemical Hygiene Plan to address the use, storage and handling of chemical hazardous materials.
